Learn about our Medical Review Board Steven Gans, MD Updated on February 09, 2020 Generalized Anxiety Disorder Overview Symptoms & Diagnosis Causes Treatment Living With In Children Jose Luis Pelaez Inc?/Getty Images Living with an anxiety disorder while you are pregnant adds stress to an already challenging time of your life. In the case of generalized anxiety disorder (GAD), chronic worry associated with the disorder may prevent you from enjoying the experience of being pregnant â" from the time you find out until long after your baby is born. If you find yourself experiencing chronic worry that feels out of your control, consider mentioning how you are feeling to your doctor or other healthcare professional. Prevalence We know that about 6 percent of the population will experience GAD over their lifetime, and during any year, about 1 to 3 percent of individuals will be living with GAD.?? The disorder is twice as common in women as men, which makes it particularly relevant to pregnancy. The rate of GAD during pregnancy has been shown to be in the range of 8.5 percent to 10.5 percent. GAD tends to go underdiagnosed and may be difficult to tell apart from normal worrying. Along the same lines, a woman who was always a worrier may develop GAD during pregnancy, possibly related to changes to do with hormones, mental state, and social obligations (e.g., leaving work, preparing for raising a family). A 2011 study showed that anxiety symptoms tend to be highest in the first trimester and decrease throughout the pregnancy.?? However, many women have GAD overlapping with other disorders such as depression, which can make them, and their unborn children, extremely vulnerable. Risks and Complications A 2015 longitudinal study of women with major depressive disorder and GAD showed that those with GAD in addition to depression had a poorer quality of life and experienced more persistent worry.?? Untreated GAD may lead to pregnancy complications such as low birth weight, preterm delivery, high blood pressure, problems with neurodevelopment of the baby, and failure to progress during labor. Once your baby arrives, you may also have trouble managing the demands of a newborn and developing a bond with your baby. Treatment Options Treatment of GAD during pregnancy may involve psychoeducation, therapy, and/or medication. Typically treatment will be tailored to the unique situation of the mother, taking into account your severity of symptoms and history of anxiety: Psychoeducation is important in early stages and during diagnosis, to help reduce denial and stigma and lead to successful treatment.Therapy such as cognitive-behavioral therapy (CBT) is most often used on its own in mild cases of GAD or in combination with medication in more severe cases. CBT may involve cognitive restructuring, exposures, relaxation training, and mindfulness therapy.Medication may be used in more severe cases. Typically a selective serotonin reuptake inhibitor (SSRI) (e.g., Prozac, Cipralex) would be prescribed. Benzodiazepines may be used for the short-term management of anxiety.Support is also an important component of coping. If you have GAD and are pregnant, seek out support from family, friends, and the community to help you cope. Can I Take Medication for GAD While Pregnant? If you have GAD and are pregnant, you may wonder whether it is safe for you to take medication. While this decision is best left to your doctor, typically medication is prescribed when the benefits are thought to outweigh the risks. In other words, if you suffer from crippling anxiety that will almost certainly negatively impact your pregnancy, medication may be a good option. SSRIs cross the placenta but are considered reasonably safe during pregnancy.?? Babies of mothers who took SSRIs during pregnancy may suffer from neonatal adaptation syndrome, with symptoms such as jitters, trouble feeding, and irritability. If youve taken benzodiazepines regularly during pregnancy, your baby will also be monitored for side effects of the medication. You may also wonder about breastfeeding. Certain SSRIs have a low concentration in breast milk â" so breastfeeding while on these medications may not be contraindicated. However, the long-term effects of these drugs on the developing child is still not fully known. You should weigh the potential benefits and risks with your doctor. A Word From Verywell You may simply wonder whether it is responsible to become pregnant if you have GAD. When successfully treated, there isnt any reason why a woman with GAD should not have children. Make a plan with your doctor on how to manage symptoms if they arise and be in contact on a regular basis to keep your anxiety in check.

What Is the Economics and Geography of Brazil
Brazil is the fifth largest country in the world in terms of population (207.8 million in 2015) and well as land area. It is the economic leader of South America, with the ninth-largest economy in the world, and a large iron and aluminum ore reserve. Fast Facts: Brazil Official Name: Federative Republic of BrazilCapital: BrasiliaPopulation: 208,846,892Ã (2018)Official Language: PortugueseCurrency: Reals (BRL)Form of Government: Federal presidential republicClimate: Mostly tropical but temperate in southTotal Area: 3,287,957 square miles (8,515,770 square kilometers)Ã Highest Point: Pico da Neblina 9,823 feet (2,994 meters) Lowest Point: Atlantic Ocean 0 feet (0 meters) Physical Geography From the Amazon basin in the north and west to the Brazilian Highlands in the southeast, Brazils topography is quite diverse. The Amazon River system carries more water to the ocean than any other river system in the world. It is navigable for its entire 2,000-mile trip within Brazil. The basin is home to the most rapidly depleting rain forest in the world, losing about 52,000 square miles annually. The basin, occupying more than 60% of the entire country, receives more than 80 inches (about 200 cm) of rain a year in some areas. Almost all of Brazil is humid as well and has either has a tropical or subtropical climate. Brazils rainy season occurs during the summer months. Eastern Brazil suffers from regular drought. There is little seismic or volcanic activity due to Brazils position near the center of the South American Plate. The Brazilian Highlands and plateaus generally average less than 4,000 feet (1,220 meters) but the highest point in Brazil is Pico de Neblina at 9,888 feet (3,014 meters). Extensive uplands lie in the southeast and drop off quickly at the Atlantic Coast. Much of the coast is composed of the Great Escarpment, which looks like a wall from the ocean. Political Geography Brazil encompasses so much of South America that it shares borders with all South American nations except Ecuador and Chile. Brazil is divided into 26 states and a Federal District. The state of Amazonas has the largest area and the most populous is Sao Paulo. The capital city of Brazil is Brasilia, a master-planned city built in the late 1950s where nothing existed before in the Mato Grasso plateaus. Now, millions of people reside in the Federal District. Human Geography Two of the worlds 15 largest cities are in Brazil: Sao Paulo and Rio de Janeiro, and are only about 250 miles (400 km) apart. Rio de Janeiro surpassed Sao Paulos population in the 1950s. Rio de Janeiros status also suffered when it was replaced by Brasilia as the capital in 1960, a position Rio de Janeiro had held since 1763. However, Rio de Janeiro is still the undisputed cultural capital (and major international transportation hub) of Brazil. Sao Paulo is growing at an incredible rate. The population has doubled since 1977 when it was a metropolis of 11 million people. Both cities have a huge ever-expanding ring of shanty towns and squatter settlements on their periphery. Culture and History Portuguese colonization began in Northeastern Brazil after Pedro Alvares Cabrals accidental landing in 1500. Portugal established plantations in Brazil and brought slaves from Africa. In 1808, Rio de Janeiro became the home of the Portuguese royalty, which was ousted by Napoleons invasion. Portuguese Prime Regent John VI left Brazil in 1821. In 1822, Brazil proclaimed independence. Brazil is the only Portuguese-speaking nation in South America. A military coup detat of the civilian government in 1964 gave Brazil a military government for more than two decades. Since 1989, there has been a democratically elected civil leader. Though Brazil has the worlds largest Roman Catholic population, the birth rate has significantly decreased over the last 20 years. In 1980, Brazilian women gave birth to an average of 4.4 children each. In 1995, that rate dropped to 2.1 children. The annual rate of growth has also decreased from just over 3% in the 1960s to 1.7% today. An increase in contraceptive use, economic stagnation, and the diffusion of global ideas through television have all been explained as reasons for the downturn. The government has no formal program of birth control. There are fewer than 300,000 indigenous Amerindians living in the Amazon basin. Sixty-five million people in Brazil are of mixed European, African, and Amerindian descent. Economic Geography The state of Sao Paulo is responsible for about half of Brazils Gross Domestic Product as well as about two-thirds of its manufacturing. While only about 5% of the land is cultivated, Brazil leads the world in coffee production (about a third of the global total). Brazil also produces a quarter of the worlds citrus, has more than one-tenth of the cattle supply, and produces one-fifth of the iron ore. Most of Brazils sugar cane production (12% of the world total) is used to create gasohol, which powers a portion of Brazilian automobiles. The key industry of the country is automobile production.

health insurance matrix HCS/235 Essay - 637 Words
University of Phoenix Material Health Insurance Matrix As you learn about health care delivery in the United States, it is important to understand the various models of health insurance to develop a working knowledge as you progress through the course. The following matrix is designed to help you develop that knowledge and assist you in understanding how health care is financed and how health insurance influences patients and providers as important foundational information for your role as a future health care worker. Fill in the following matrix. Each box must contain responses between 50 and 100 words using complete sentences. Include APA citations for the content you provide. The employee, or beneficiary, paid the difference. Then, the health insurance company pays the provider directly. Example: HMOs have the strictest access structure, called a gatekeeper model, where patients must have a primary care physician (PCP) through whom all care is routed. PCPs decide which diagnostic tests are needed and control access to specialists through referrals, deciding when it is necessary for a patient to seek more expensive specialty care (Barsukiewicz, Raffel, Raffel, 2010). Example: HMOs are usually the least expensive health plans, offer predictable costs for health care, the least administrative paperwork, and cover preventive care (Barsukiewicz, Raffel, Raffel, 2010). However, HMOs also restrict direct access to specialists by requiring referrals by a PCP, requiring patients to see a provider in the HMO network, and often not covering more costly procedures or care options, because care is managed to control excessive or unnecessary care. Providers gain if they provide less care (Austin Wetle, 2012). This incentive could affect patient-provider trust. Example: Advantages of HMOs are that a known amount of revenue is guaranteed and the patient population number is fixed (Austin Wetle, 2012). Usage That Provokes Blackboard Moments Usage That Provokes Blackboard Moments Usage That Provokes Blackboard Moments By Maeve Maddox The comments on my post about writing dates with or without terminals got me thinking about the way everyone who speaks English reacts strongly to at least one word or point of usage. The different ways that people write a date seem to excite curiosity without making anyone angry, but sometimes words or expressions evoke annoyance so intense as to constitute rabid aversion. (Im thinking of the responses provoked by my article on couldnt care less.) By a blackboard moment I mean a physical reaction similar to what we feel when the teachers hand slips and we hear a fingernail scrape against the board. Here are some of the words, pronunciations, spellings and expressions that produce blackboard moments of various intensities in me. (The preferred form is in parentheses.) standing on line (standing in line) light something on fire (set something on fire) Me and my friends swim. (My friends and I swim.) in hopes of (in the hope of) pronouncing the word pecan with a long e and a short a: /pee can/ (instead of with a schwa and the a of father: /pe kahn/) pronouncing the t in Bill Clinton (he pronounces his name with a glottal stop: /klin?n.) seperate (separate) dalmatien (dalmatian) shepard dog (shepherd dog) cemetary (cemetery) its tail (its tail) In that incidence he was right. (In that instance he was right.) Do you want some sandwich? (Do you want part of a sandwich?) How about you, Gentle Reader? Why Renting a Flat in Germany Is Totally Common
Why Renting a Flat in Germany Is Totally Common Although Germany has got the most successful economy in Europe and is basically a wealthy country, it has also got one of the lowest homeownership rates on the continent and is also way behind the US. But why do Germans rent flats instead of buying them or even build or buy a house? Buying their own accommodation is the goal of many people and especially families all around the world. For Germans, it might seem that there are things way more important than being a homeowner. Not even 50 percent of the Germans are homeowners, whereas over 80 percent of the Spanish are, only the Swiss are even renting more than their northern neighbors. Lets try to track the reasons for this German attitude. Influence of World War II Like many things in Germany, the tracking of the attitude to rent reaches back to the Second World War. As the war ended and Germany signed the unconditional surrender, the whole country was rubble. Almost every bigger city was destroyed by the British and American Air Raids and even the smaller village had suffered from the war. Cities like Hamburg, Berlin or Cologne where nothing but a big pile of ashes. Many civilians became homeless because their houses were bombed or collapsed after the fights in their cities, over 20 percent of all housing in Germany was destroyed. That was why it was one of the first priorities of the new built West-German government in 1949 to prove every German a safe place to stay and live. Therefore, big housing programs were started to rebuild the country. Because the economy was also laying on the ground, there was no other opportunity than having the government put in charge of new housings. For the newborn Bundesrepublik, it was also very important to give the people a new home to face the opportunities communism promised just on the other side of the country in the Soviet zone. But there was, of course, another opportunity coming with a public housing program: Those Germans who hadnt been killed or captured during the war were mostly unemployed. Building new flats for over two million families could create jobs that were urgently needed. All this lead to success, the lack of housings could be reduced during the first years of new Germany. Renting Can Just Be a Good Deal in Germany This leads to the fact that Germans today just as their parents and grandparents have reasonable experiences with renting a flat, not only from a public housing company. In the major cities of Germany like Berlin or Hamburg, most of the flats available are in public hand or at least managed by a public housing company. But besides the big cities, Germany has also given the private investors the opportunity to own properties and rent them out. There are many restrictions and laws for the landlords and tenants they have to follow which proves that their flats are in a good condition. In other countries, rental flats have the stigma of being run down and mainly for poor people who cant afford to own an accommodation. In Germany, there are none of those stigmas. Renting seems just as good as buying - both with advantages and disadvantages. The Laws and Regulations Made for Renters Talking about the laws and regulations, Germany has got some specials that make a difference. For instance, there is the so-called Mietpreisbremse, which passed the Parliament. In areas with a strained housing market the landlord is only permitted to increase the rent up to ten percent above the local average. There are a lot of other laws and regulations which lead to the fact that the rents in Germany - compared to those of other developed countries - are affordable. On the other side, German banks have high preconditions for getting a mortgage or a loan to buy or build a house. You just wont get one if you dont have the right sureties. For long term, renting a flat in a city can therefore be a better opportunity. But there are of course some negative sides of this development. Like in most other western countries, the so called gentrification can also be found in Germanys major cities. The good balance of public housing and private investment seemed to tip over more and more. Private investors buy old houses in the cities, renovate them and sell or rent them out for high prices only wealthy persons can afford. This leads to the fact that normal people can no longer afford living inside the big cities and especially young people and students are stressed to find a proper and affordable housing. But thats another story because they couldnt afford buying a house either.
